In consideration for permission to participate in Vacation Bible School activities hosted on the property owned or controlled by CtK. I agree to indemnify, defend, and hold harmless Christ the King, and any of its employees, agents, members, or volunteers from any liability resulting from my child’s presence or my child’s participation in any activity. I agree to release, discharge, and commit not to sue CtK, any of its employees, agents, members, volunteers, invitees or licensees for any loss or damage, or any claim as a result of any injury or loss of life due to any negligent act or omission by any party while my child is on the property owned or controlled by CtK; or participating in activities involving CtK, its employees, agents, members, or volunteers; or during transportation to or from any of these activities. This waiver does not preclude action against specific parties in events of willful criminal harm.

I accept full responsibility of any and all risks associated with my child’s attendance and participation in the activities discussed above. I consent to both first aid and emergency medical attention in the event of my child’s injury or incapacitation and agree to release and hold harmless any party from any damages or liability resulting from the provision of first aid or emergency medical attention including movement and transportation to a medical care facility.
